Rehabilitation Case Management

Our case managers work closely with clients, their families, solicitors, insurers and other health professionals to identify solutions to meeting an individual's needs.

After receiving a referral we conduct a full assessment of each client's needs including:
  • A thorough consideration of all relevant factors such as family situation, external environment etc.
  • Identification of key aims and goals.
  • Matching aims and goals to a case manager who is suited in terms of personality and experience.
  • Development of an action plan including recommendations as to how all aims can be achieved.


We provide the client with an individualised, financially apportioned case management report An appropriate support package will be recommended (incorporating all aspects of the person's life; from home situation to employment to social needs to education), will be developed and continuously monitored/evaluated.


We then work with the client to meet their aims and goals including such supports as:

Providing assistance with the recruitment, training and support of personal support staff.
Assessing for the purchase of appropriate specialist equipment or environmental adaptations.
Assisting in liaising with appropriate agencies or external sources to source and justify funding.
Advocating for, and providing support to, our client's and their families.
